Objective: To secure employment as a Case Manager in the state of California. Having six years of active Hospital Nursing and four years of Mental Health Case Management in the state of Mississippi, seeking change due to relocation forced by circumstances.
Experience:
2004 to Present: Hospital Case Manager, County Mental Health Hospital, Lafayette, Missouri. As Case Manager, responsible for Outpatient Case Management of 70 discharged patients. After a six month period of on the job training under an experienced Case Manager, was given independent responsibility and have carried out responsibilities to the entire satisfaction of the Management. Have successfully rehabilitated twenty patients to be independent of Case Management.
1998 to 2003: Senior Nurse, Bolivar County Hospital, Cleveland MI. Joining as Trainee Nurse moved up to Senior Nurse in three years and was Nurse In Charge of the Trauma Center of the Hospital prior to moving on to a Case Management Career.
Education:
Bachelor’s Degree in Nursing, Chamberlain College of Nursing, St. Louis, Missouri.
Certifications:
Certified Case Manager by the Commission for Case management Certification (CCMC).
Certified Rehabilitation Nurse by the Association of Rehabilitation Nurses (ARN)).
Current License to practice as a Registered Nurse.
